A Catholic Advance Health Care Directive puts those decisions in the hands you trust, not the hospital or the courts, so the faith you live by speaks for you when you cannot, and the witness you hope to pass on is honored at the bedside.

  • Names the person you trust and records your wishes on care, food and water, and end-of-life decisions, in line with Catholic moral teaching.

  • Asks that a priest be called for the Anointing of the Sick, Confession, and Holy Communion as Viaticum, the sacramental care standard forms leave out.

I direct my agent and my care providers to notify a Catholic priest so that I may receive the Anointing of the Sick, sacramental Confession if I am able, and Holy Communion as Viaticum.
